Hello. A few items I sent to Amazon FC warehouses were missing. The reason is that the barcode cannot be scanned. I have never experienced such a problem with any of my shipments so far. I have heard from another seller (who no longer posts on the NSFE) that the new scanners that Amazon is switching to do a poor job with glossy labels. Since he had his FNSKU printed on the item, he had to revamp his packaging process (this is a big seller).
If your labels are glossy, try switching to a more matte finish label.
If that's not it, I have no idea.
